The table below confirms the documents that can be accepted to confirm your Identity and Address. Please note the document must be an original and the same document can not be used for identification and address verification. Please bring in one document from each section.

A duplicate licence is acceptable but a provisional licence is not. • Photo card — a duplicate licence is acceptable. A provisional licence is only acceptable for individuals aged under 18. |
| Passport | This must be a current full passport, and must not have expired. |

| HM Revenue & Customs (HMRC) (Formerly Inland Revenue) Tax notification |
The documentation must have been issued in the current tax year, must be in your name and show your current address in full. Acceptable examples: • PAYE Coding Notice • HMRC Notification of Working Family Tax Credit • HMRC Notification of Children's Tax Credit • HMRC Notification of Disabled Persons Tax Credit • HMRC Tax Calculation P45's and P60's are not acceptable. |

• Letter must be addressed to the applicant or HSBC Bank. • Letter must be dated within the last six months and on official headed paper. • Letter must include a contact name, address and telephone number. If you are in doubt a letter is genuine, contact the issuer for confirmation. |
| Letter from Nursing & Midwifery Admissions Service (NMAS) confirming acceptance onto a programme |
• Letter must be addressed to the applicant. • Letter must be dated within the last six months and on official headed paper. • Letter must include a contact name, address and telephone number. |
| Bank, building society or credit union statement or passbook | Containing current address and showing activity (ie, customer transactions) within the last four months. |
| Council tax, local authority or water rates bill | This must be valid for the current year |
